Required Documentation

The following documentation may be required to support your online Request for Waiver of the Occupational Lead Poisoning Prevention Fee. All documentation should be provided for the previous calendar year. You will be prompted towards the end of your online application to submit required documentation to support your waiver request.

Construction, Painting, Demolition Work, and Other Construction Trades
  • List of jobs for the previous calendar year, including tasks, job duration, site address and date of original construction
  • Pre-job lead testing results
  • Date(s) on which employees worked on a pre-1978 structure or metal structure
  • Subcontractor contact information
  • Safety data sheets (SDS) or chemical composition for materials used
  • Invoices for materials purchased
  • Estimates of solder and brazing materials use
  • Contract language showing party(ies) responsible for lead testing and/or abatement for work on pre-1978 structures
Metal Work
  • SDS or chemical composition for materials used, altered, or disturbed
  • List of metal alloys used
  • Invoices for materials purchased
  • Estimates of solder and brazing materials use
Retailers, Wholesalers and/or Distributors
  • SDS or chemical composition for solder
  • Invoices for solder purchased
  • Estimates of solder use
Manufacturing
  • SDS or chemical composition for materials used, altered, or disturbed (e.g., plasticizers, stabilizers, resins, paints, adhesives, sealants, rubber, pigments, glazes, chemicals, metals, or other separate raw ingredients)
  • Invoices for materials purchased
  • Estimates of solder and brazing materials use
Automotive Services (including Auto Body)
  • SDS or chemical composition for powder coatings, paints, brazing, welding, or solder materials
  • Invoices for materials purchased
  • Estimates of solder and brazing materials use
  • Pre-job lead testing results for automotive or other paints altered or disturbed
  • Date(s) on which employees worked on vintage cars or automobiles with lead-based paint
Detective, Guard, Armored Car, Other Security Services
  • Dated official firearms policy

Printing or Copying
  • SDS or chemical composition for materials used (inks, colorants, etc.)

Handling, Processing, and/or Recycling Material
  • Specific materials handled, sorted, or otherwise processed or recycled

Communication Services or Equipment

  • Invoices for solder purchased
  • Estimates of solder use
If your Company was Out of Business for the Previous Calendar Year
  • Copy of form filed with the California Secretary of State to dissolve, surrender, or cancel your business entity, or a letter from an attorney stating this
  • Examples of acceptable forms include: Certificate of Election to Wind Up and Dissolve (Form ELEC STK or ELEC NP), Certificate of Dissolution (Form DISS STK, DISS NP, DSF STK, or LLC-3), Certificate of Cancellation (Form LLC-4/7 or LLC-4/8), Certificate of Dissolution (Form GP-4), Notice of Change of Status (Form LLP-4), or Certificate of Right to Transact Intrastate Business (Form SURRENDER-CORPORATION)
